New Patients:

Please complete this packet prior to your appointment. The last two pages of this packet describe our practice policies. You will also need to bring your insurance card and a photo identification card (such as a drivers’ license) to your appointment.

Please remember that you are responsible for obtaining a referral from your primary care physician if your insurance requires a referral.  If you do not have your referral your appointment will be rescheduled.

The release of any medical records requires a written request from the patient (or guardian).  Please fax the form below to 703-369-9240 (or bring it to any of our offices in person) to request medical records.  There is a fee for copying and mailing medical records.  You will be contacted by our service affiliate to request payment for these records.
