**If your insurance needs a referral, please bring it with you to your visit or you will not be able to see the doctor.  As always, it is the patient''''s responsibility to obtain their own referral.

***Co-pays must also be paid before you see the doctor.  Insurance carriers require that co-pays be obtained at the time of the visit.

****Workers Compensation visits must be authorized before the patient can be seen.  Please contact our office before your visit to have it authorized by your Workers Compensation company.

 
  • CareFirst BlueChoice HMO

    As of October 31, 2007 we will no longer be a participating provider with CareFirst Blue Choice HMO. 

    However, we will continue to accept CareFirst PPO.
  • FirstHealth Effective December 1, 2004 we will no longer be accepting FirstHealth.
